Advisor Checklist

Each Crew Advisor should be certain to have the following documents for the crew:

• Completed Philmont Crew Roster (this is the multipart form received with the TREKS-2002 Itinerary book).
   
• Completed Talent Release Roster (this is the form received with the TREKS-2002 Itinerary book and must be signed by parents of participants younger than 18).
   
• Proof of current CPR certification (original & copy of certification card for at least one crew member).
   
• Proof of current First Aid training (original & copy of certification card for at least one crew member).
   
• Philmont Health and Medical Record for each crew member (must be on original Philmont 2002 Medical form) with insurance information (copy of insurance card), parent/guardian and physician signatures.

The Crew Advisor should have the original copies of these documents.  A complete copy of them should be given to the Tour Director as soon as possible.  The Crew Advisor should also complete the Crew List and give it to the Tour Director with the other information.
