Advisor Checklist

Each Crew Advisor should be certain to have the following documents for the crew:

• Completed Philmont Crew Roster (this is the multipart form received with the TREKS-2003 Itinerary book).
   
• Proof of current CPR certification (copy of certification card for at least one crew member).
   
• Proof of current First Aid training (copy of certification card for at least one crew member).
   
• Philmont Health and Medical Record for each crew member with insurance information (copy of insurance card), parent/guardian and physician signatures.
   
• Philmont Talent Release form.  This must include information about each crew member and must be signed by the adult crew members and the parents of the youth crew members.

The Crew Advisor should have the original copies of these documents.  A complete copy of them should be given to the Tour Director as soon as possible.  The Crew Advisor should also complete the Crew List and give it to the Tour Director with the other information.
