Each Crew Advisor should prepare a binder with the following documents for their crew:

Multipart form received in March, 2006 with the TREKS-2006 Itinerary book.

Turn in original Day 1 Finance check-in.

CPR & First Aid Certification cards. At least one person per crew must be certified in First Aid and one in CPR.

From organization teaching the courses.  They must be valid though the end of the trek.

Philmont Health and Medical Record form.  Verify that this has been properly signed by qualified health professional, parent/guardian, participant.

Distributed in October, 2005 - MUST USE THE ORIGINAL PHILMONT COPY OF THE 2006 FORM - NO COPIES ARE PERMITTED.  DO NOT SEPARATE THE PAGES OF THE MEDICAL FORM--COMPLETE FORM IS REQUIRED.

Turn in original Day 1 Medical re-check.  Will be returned a check-out Day 12.

Copy (only the actual pages that have information filled in and/or signatures)

Medical Insurance Card

Make copy, staple to Health & Medical Record form.  If family has no insurance, attach a note indicating this and mark the appropriate place on the Health & Medical Record form.

Turn in with Health & Medical Record form during Day 1 Medical re-check.  Will be returned a check-out Day 12.

Philmont Talent Release form
Requires parent/guardian signature if participant is under 18 years of age.

Received in March, 2006 with the TREKS-2006 Itinerary book.

Crew Emergency Contact Listing

Not used by Philmont - this is to provide the Contingent Advisor with emergency contact information.

Original or Copy

The Crew Advisor should have the original copies of all these documents.  These should be in a binder that the Crew Advisor takes to Philmont (it does not go on the trail -- it is left in the Crew locker). 

A complete copy of the documents should be given to the Contingent Advisor as soon as possible.  They MUST be turned in at the final Contingent meeting.  The busses will not leave Austin with incomplete paperwork!

Remember:

Every participant is required to be a registered member of the Boy Scouts of America. 

All adult participants (18 years of age or older for Boy Scout Troop based crews and 21 or older for Venturing Crew based crews) must be current in Youth Protection Training (within last 3 years).  The on-line training course may be taken by clicking the button below:

Youth Protection Training

If a participant takes the on-line training course, they can print a copy of the certification card that is e-mailed to them on completion of the course.

It is the Crew Advisor's responsibility to insure that all adults are properly registered and are current in Youth Protection Training.
