Before departing Philmont, your crew leadership completed surveys that were relative to your experience while at Philmont.  This survey relates to the Capitol Area Council’s Philmont Contingent — not the Philmont experience itself.  We appreciate your candid response to this survey — it will help us improve in the future.  You may or may not identify yourself but, in any event, all responses will remain confidential. 

Some of the questions on the survey form are applicable only to the adult Crew Advisors (Crew Advisor Binder).  Others are applicable to any participant.  Please respond to those questions that apply to you or that you have personal knowledge of.

On this form, check the boxes if your response is "Yes", leave them blank if "No."  Where a ranking is available, respond "1" for "Strongly Disagree" through "5" for "Strongly Agree."
